We have a team of physicians and nurse practitioners that provide specialized medical care for patients who transfer from one care center to another, even if they require long-term care in a skilled nursing facility. Our providers are specialized in internal medicine, family medicine, geriatrics, and hospice and palliative medicine. Their goal is to provide quality care specific to your health care needs and long-term goals.
If you choose one of the skilled nursing facilities listed in this brochure, the facility's staff will coordinate your care and contact us directly if they assign one of our post-acute care providers to you.
Members of your UPMC post-acute care team will oversee your medical needs at the skilled nursing facility and maintain communication with your physician, as well as assist you with planning any medical care you may need after you leave the skilled nursing facility.
