To safeguard your privacy, you must complete and sign a protected health information (PHI) release form before we can send your records.
On the form, you can let us know:
Doctors or others who want access to your medical records must also complete the HIPAA-compliant release form.
UPMC charges modest, cost-based fees for copies of medical records. We use the Pa. Department of Health's charges for medical records fee schedule.

Every UPMC patient can request a change to their health record.
To amend your record, complete the UPMC patient amendment to PHI form and mail it to the proper medical records department. The process may take up to 60 days.
Please note, we may consult your doctor before making changes to your record. UPMC can also deny the request if we deem your record correct and complete.
