Planning ahead for the day of discharge will make the process go as smoothly as possible for you.
Making preparations for transportation and assistance the night before an anticipated discharge will allow you time to concentrate on your discharge instructions and final preparations for departure. Make sure you have confirmed with the staff what time you will be discharged.
If you need assistance to arrange transport for special needs, ask your nurse to contact the Social Work and Discharge Planning Department. The person coming to transport you home may drive to the discharge area at the main entrance of the building. Patients are to be escorted to the main entrance by hospital staff or volunteers.
